- Helps with level of care determinations
- Refers to programs related to health education, nutrition, transportation, and income support
- Screens for Medicaid nursing facility and home and community-based service waiver programs
- Offers case management for older adults and consultations and seminars for caregivers
- Assists with applications for the Homeowners' and Renters' Tax Credit program
